Parent or Guardian Consent Form

I, the parent or Gaurdian of the above named applicant, give my consent for my minor child to volunteer with InspireHealth.

I understand that my minor child may or may not by supervised by an adult. If the minor is under 13 I understand that I, or another adult, must be present at all times during thier volunteer activity.

I understand that my minor child is to participate in an orientation and training prior to thier volunteer placement.

I understand any concerns about about the volunteer position, policies or employees must be directing to the volunteer coordinator.

I understand that it is my responsibility to inform the volunteer coordinator of any changes concerning any limitations to volunteering and emergency contact information.
